ACCIDENT CHECKLIST
Print this checklist and place it in your glove compartment. After a traffic collision, you should normally take
the following steps:
1. Contact Police: Contact the police, sheriff or highway patrol. Insist that the officer take a report. 2. Involved Person Identification: Obtain the following information from each and every person involved in the collision: Name: Address: Vehicle license plate number: Driver's license number: Insurance information: 3. Witness Identification: Obtain from each witness, their: Name: Address: Telephone Number: 4. Photograph: Take photographs of the following: Scene:
The collision/accident scene;
Vehicles:
All vehicles involved (before
repairs are made)
Your
Injuries: Any visible injuries (cuts, bruises,
swelling).
5. Don't speak to anyone
about the accident or injuries except:
Police
Officer(s): Talk to police officers about accident and your
injuries.
Your doctor(s): Talk to your medical professionals (See #8 below). Your lawyer: Speak to your lawyer (It's confidential). 6. Insurance Adjuster: Don't speak to an insurance adjuster about the incident without first receiving legal advise. Anything you say can and will be used against you! 7. Promptly seek medical attention. 8. Tell your physician or doctor: How the injury occurred ("I was involved in an intersectional traffic collision."); and All your symptoms and complaints. Report
memory problems, confusion or disorientation to your
physician, chiropractor, or healthcare practitioner even if it seems minor.
10. In California, you must report your accident to the DMV within ten days if: a. More than $750 in property damage to any person's vehicle or other property; or b. You or any other person sustained any injured whatsover, or death. Use DMV Form SR1. 11. Call an experienced personal injury attorney to receive a consultation.Copyright
